I recommend a dedicated mail host like Tuffmail (tuffmail.com) -- unless you enjoy running your own mail server. Tuffmail has a very configurable (and very unix-admin-friendly) spam+av filtering setup which can be very tuned to be very aggressive (using the block lists, greylisting, network-based rejections, etc.). They also provide spam/ham training folders. For me it was either use them or run my own mail server, but I'd really rather pay someone to do that for me.
@1: I have a pretty fine tuned anti-spam filtering system on my mailserver. Works pretty well and by now it adapted already to that funky christmas spam :-)
